Out of six children files reviewed, two children did not have all the required immunizations. One child was missing proof of 1 of 4 Dtap, 1 of 3 HepB, and 1 of 4 PCV. Another child was missing proof of 2 of 4 Dtap, 1 of 3 PCV, 1 of 3 Hep B, 1 of 3 Hib, and 1 of 4 PCV.
